Depression Questionnaire – This questionnaire is for children age 11-21 years of age and will help the doctors understand your t(w)eenager’s stress and mental development. PHQ 2 and PHQ 9
Post-Natal Questionnaire – This questionnaire is meant for the mother/caregiver and will help the doctor’s understand the factors that will help/hinder the baby’s development. This questionnaire is for babies 0 – 1 year of age and the mother/caregiver should fill this form. PHQ 2 and PHQ 9
Medical Release of Information – This form is to release medical records to the requesting person. Please note the parent or legal guardian has to sign this form. If the patient is 18 years or older, the patient has to sign this form.Medical Release of Information
Important Information Before Booking
Please review the following before scheduling your appointment:
Existing Patients Only: Online booking is available exclusively for established patients of Huntington Plaza Pediatric Group. If you are a new patient, please contact our New Patient Coordinator to schedule your first visit.
Sick/Urgent Visits Only: This online booking option is reserved for emergency and sick visit appointments (15-minute visits) only. For annual well-child checkups, physicals, or routine wellness visits, please call our office directly to schedule.
Early Arrival Required: Please arrive at least 15 minutes prior to your scheduled appointment time to complete any necessary paperwork and check-in procedures.
No-Show Policy: A $50 no-show fee will be applied if an appointment is missed without prior notification. As we utilize a third-party scheduling service, timely cancellations help us serve all our families better. Please contact our office as soon as possible if you need to cancel or reschedule.
